My son will be starting Pre K soon, and I have done a delayed vaccinating schedule til recently. I have done extensive research and have decided I am 100% against getting him an MMR, Chicken Pox, and Hep B shot at this time. For him to start pre k will I need a form from the school, or is there a place online to print one out? Any information on going about this is greatly appreciated.
My kids dr gave us our exemption form.

Michigan has philosophical exemption too. Just mark that. They don't question it. You fill out the form, send it to the state, the state should send the exemption letter to his school.

Wow that's even easier than ours! We have to mark a reason as well. But a doc note is needed if you mark medical.
I feel like an idiot with this stuff, but I printed out the waiver, filled it out. Do I send it somewhere, take it to the health department, the school, or my doctor?
